Аннотация: It is now well-established that sustained activation of the central nervous system, in particular a specific neuro-hormonal and emotional activation pattern called active distress, is associated with the development of CHD. Recent socio-epidemiologic research showed that active distress is often triggered by specific job conditions. The present study investigates associations between stressful job conditions and the development of CHD on the basis of the effort-reward imbalance (ERI) model. This model assumes that active distress is a consequence of the mismatch between high (extrinsic or intrinsic) effort spent and low reward (e.g. money, esteem, status control) obtained at work. Results from two studies confirm this hypothesis. Indicators of ERI successfully predicted new coronary events in a 6.5 years prospective study among middle-aged blue-collar men (multivariate odds-rations (OR) 3.4-4.5). In a cross-sectional study in middle-aged male middle-managers ERI was associated with cigarette smoking, elevated atherogenic lipids, and hypertension (OR 3.3-6.8). Results so far underline the independent role of ERI in the development of CHD. In conclusion, preventive activities which combine a reduction of stressful work conditions with improvement of individual coping behavior are most promising.
